“医改”的政策学习与政策工具——中国公立医院改革与新加坡经验

摘要(Abstract):
中国正在进行的公立医院改革应该立足中国实践基础,更多借鉴亚洲国家的改革经验。比较政策分析应该导向更有效的政策学习。新加坡1980年代开始的公立医院改革,与筹资改革相配套,以公司化为主要内容,在保证政府对医院产权所有的基础上,采取一系列政策工具,重组医疗领域的各种激励机制。在财政投入方式、供方支付方式、法人治理机构、政府监管方式和信息披露机制方面取得了显著成绩,值得中国借鉴。新加坡的经验说明,对于医疗服务这个存在天然性市场失灵的领域,要达致资源合理配置,让市场发挥作用,必须以增强而不是削弱政府的调控力为前提。政府应在保持公有产权和必要财政投入的基础上,合理运用各种政策工具。
